ERCP-induced perforation: review and revisit after half a century

Background: Endoscopic retrograde cholangiopancreatography (ERCP) is an invasive procedure. We aimed to investigate ERCP-induced perforations at our institution and conduct a comprehensive review of literature on (EIP) since the introduction this procedure as therapeutic intervention. Methods: This was case-control study, in which charts all patients diagnosed with duodenal perforation were reviewed compared control group without perforation. Patient’s sociodemographic clinical data, including ERCP procedure-related gathered. Results: A total 996 procedures performed; only 13 proved have EIP. Obstructive jaundice most common indication for ERCP. The main predisposing factor difficult cannulation (P = 0.003). In total, five required surgical treatment; majority them had type I perforation, whereas IV who treated conservatively. overall mortality rate 15%, slightly higher rate. Conclusions: Fifty years after therapy, it remains that carries significant morbidity mortality, even skilled hands or high- volume units. Conservative management yields favorable outcomes selected patients.
